Description of Course:
Health Records Exchange are foundational to continuity of care and the reason for Electronic Health Records (EHR), yet post-acute and home- and community-based care providers often operate in isolation from hospital and primary care EHR.
This webinar examines why interoperability breaks down across the post-acute continuum and provides workflow-first, patient-centered strategies to close those gaps. Participants will learn practical, regulatory-aligned approaches to strengthen care transitions, improve data sharing, reduce avoidable readmissions, and elevate operational performance. Attendees will leave with actionable tools to build safer, more connected systems that drive better outcomes for patients, providers, and organizations.

Registrants must be individually logged in for the full session to receive CE hours and certificate. BRN certificates will be available on your member dashboard after the conclusion of the training. (quiz and evaluation must be completed first). This training is only approved for BRN CEs.

BOARD REFUND POLICY
In order to receive a refund, cancellations for QCHF/CAHF classes must be made five (5) business days (Monday-Friday) prior to the beginning of the course. Cancellations must be in writing to cmerced@cahf.org. Transfer of registration is done ONLY if notified in writing prior to the start of the class.
