Description of Course:
This session unpacks CMS’s revised definition of a fall with major injury and the far-reaching implications of the October 2025 RAI Manual updates. Using real-world examples and OIG findings, participants will explore how documentation gaps have contributed to under-reporting and inaccurate Quality Measure data. The training provides practical strategies for accurate MDS coding, interdisciplinary communication, and data integrity—linking compliance directly to resident safety and quality outcomes.
Learning Outcomes:
Clarify CMS’s Revised Definition
Explain how the October 2025 RAI Manual redefines a fall with major injury and aligns documentation expectations with OIG findings showing underreporting (43% unreported).
Strengthen Assessment and Documentation Practices
Teach nurses and IDT members how to accurately code Section J1900–J1900C, correlate chart documentation, and prevent data integrity errors that impact Quality Measures and Five-Star ratings.
Connect Falls Data to Quality and Culture
Demonstrate how improved fall documentation supports transparency, drives quality-improvement initiatives, and reshapes the facility’s safety culture—from reactive to proactive prevention.
CE Hours (Requested):
BRN - 1 Hour
Registrants must be individually logged in for the full session to receive CE hours and certificate. BRN certificates will be available on your member dashboard after the conclusion of the training. (quiz and evaluation must be completed first). This training is only approved for BRN CEs.
Audio/Visual:
Virtual training experiences will happen in real time on the date and time advertised. They will require a Wi-Fi connection, a laptop/computer and/or a smartphone. For the best experience, we highly encourage you to participate in a distraction-free environment.

Handouts/materials will be available in the chat box during the training.
BOARD REFUND POLICY
In order to receive a refund, cancellations for QCHF/CAHF classes must be made five (5) business days (Monday-Friday) prior to the beginning of the course. Cancellations must be in writing to cmerced@cahf.org. Transfer of registration is done ONLY if notified in writing prior to the start of the class.
