Description of Course:
Learn how AI and interoperability are transforming care transitions beyond discharge. This session explores practical strategies skilled nursing facilities can use to close the 30-day transition gap, reduce readmissions, and improve coordination across all providers.
Who Should Attend:
Administrator, Director of Nursing (DON), Assistant Director of Nursing (ADON), Quality Assurance Nurse, MDS Coordinator
Learning Objectives:
1.Identify common breakdowns in the 30-day post-discharge transition.
2.Understand how interoperability gaps affect care coordination.
3.Recognize the limitations of generic predictive AI in addressing dynamic risks during post-discharge transitions.
4.Learn strategies to improve communication across the care continuum.
5.Explore emerging models for AI-enabled care coordination.
CE Hours (Requested):
BRN & NAB - 1 Hour
Registrants must be individually logged in for the full session to receive CE hours and certificate. BRN certificates will be available on your member dashboard after the conclusion of the training. (quiz and evaluation must be completed first). This training is only approved for BRN CEs.
This program has been submitted (but not yet approved) for Continuing Education for XX total participant hours from NAB/NCERS. Contact member.services@cahf.org for further information.
Audio/Visual:
Virtual training experiences will happen in real time on the date and time advertised. They will require a Wi-Fi connection, a laptop/computer and/or a smartphone. For the best experience, we highly encourage you to participate in a distraction-free environment.
Materials Provided:
Handouts/materials will be available in the chat box during the training.
BOARD REFUND POLICY
In order to receive a refund, cancellations for QCHF/CAHF classes must be made five (5) business days (Monday-Friday) prior to the beginning of the course. Cancellations must be in writing to cmerced@cahf.org. Transfer of registration is done ONLY if notified in writing prior to the start of the class.
